Community Based Rehabilitation - Ongoing Project
The project reaches out to the rural community in Bangalore Rural district to bring in awareness, knowledge and prevention of disability by providing required information to all the homes, schools, government hospitals, etc. This program also identifies individuals with disabilities, provides them with required knowledge and home based therapy. Additionally, this program trains the community to do the same.
Conduct camps to educate and prevent disabilities by providing information about nutrition/immunization and care for pregnant women/young/infant child.
Visit houses/government schools/play homes/health&Panchayat centers to identify individuals with disabilities/health problems. Provide them appropriate services/referrals to relevant organizations/hospitals/specialist.
Train the community about rights/facilities available for individuals with disabilities and provide them with knowledge/coping sk

65+ play homes and government school visits
170+ home visits are done
65+ children referred to other organization and special schools, out of this 20+ are enrolled in Shristi
12+ are getting home based therapy, two physiotherapy and special education sessions per week .

I also certify that the organization and I will abide by the terms and conditions of the
grant/fund as established by granter/funder. I further certify that all received funds/grants
will be used exclusively for the purpose stated in this proposal unless I normally request and
receive prior authorization from funder for any changes. Further, I understand that if the
funding is granted for the project described above it will be for an initial period of ONE year
and can be extended, if necessary, by renewing the proposal. Also, regular status reports on the
project will be provided by the organization to funder for the duration of the
